The Protein Creatinine Ratio (PCR) 24-hour urine test measures the amount of protein compared to the amount of creatinine excreted in urine over a 24-hour period.
Protein is normally present in very small amounts in urine. Higher levels may indicate kidney damage or disease. Creatinine, a waste product of muscle metabolism, helps normalize protein measurements and account for variations in urine concentration.
This test provides a more accurate assessment of proteinuria (excess protein in the urine) than a simple spot urine test.

    Why It’s Done:
    The 24-hour PCR test may be recommended for the following reasons:
    • Detect and monitor kidney disease: Such as diabetic nephropathy, glomerulonephritis, or hypertensive kidney disease.
    • Assess kidney function: Especially in patients with conditions like diabetes, hypertension, or autoimmune diseases.
    • Evaluate severity of proteinuria: Helps guide treatment decisions and monitor disease progression.
    • Screen for preeclampsia: In pregnant women showing signs of high blood pressure and protein in the urine.
    • Monitor effects of treatments: For kidney disorders or systemic illnesses affecting the kidneys.

    Preparation:
    • No fasting required: You can eat and drink normally unless your doctor advises otherwise.
    • Medication disclosure: Inform your healthcare provider about all medications and supplements, as some can affect urine protein levels.
    • Follow collection instructions carefully:
    o Discard the first urine sample of the day (start with an empty bladder).
    o Collect all urine passed over the next 24 hours, including the first urine the next morning.
    o Store the urine container in a cool place, preferably refrigerated.
