The Lithium Serum Test measures the amount of lithium in the blood. Lithium is a medication primarily used to treat and prevent episodes of mania in people with bipolar disorder. Because lithium has a narrow therapeutic range, meaning the difference between a therapeutic and toxic dose is small, regular monitoring is essential to ensure safe and effective treatment.

The test is usually performed 6–12 hours after the last lithium dose (commonly referred to as a “trough level”) to get the most accurate reading.

    Why It’s Done:
    This test is used to:
    • Monitor lithium levels to ensure they are within the therapeutic range.
    • Prevent lithium toxicity, which can cause serious side effects.
    • Assess treatment compliance and effectiveness in individuals taking lithium for bipolar disorder or other psychiatric conditions.
    • Investigate symptoms of toxicity, such as:
    o Nausea and vomiting
    o Tremors
    o Confusion
    o Muscle weakness
    o Poor coordination
    o Seizures (in severe cases)

    Preparation:
    • Timing is critical: Blood should be drawn 12 hours after the last dose of lithium unless instructed otherwise by your healthcare provider.
    • Do not skip or adjust your dose without medical advice.
    • Inform your doctor and lab technician about:
    o The exact time of your last lithium dose.
    o Any other medications you are taking, as some can affect lithium levels (e.g., diuretics, NSAIDs, ACE inhibitors).
    • No fasting is required.
