    Why It’s Done:

    This test is typically ordered to:
    • Diagnose Cushing’s syndrome (overproduction of cortisol).
    • Help identify Addison’s disease (underproduction of cortisol).
    • Evaluate symptoms such as unexplained weight gain, muscle weakness, high blood pressure, excessive hair growth, irregular periods, fatigue, or low blood sugar.
    • Monitor adrenal gland function or effectiveness of treatment for adrenal disorders.

    Preparation:

    • No fasting is required, but follow your healthcare provider’s instructions carefully.
    • Avoid vigorous physical activity and stress before and during the collection period, as these can increase cortisol levels.
    • Inform your doctor of all medications and supplements, especially:
    o Corticosteroids
    o Oral contraceptives
    o Anti-seizure drugs
    • You will be given a large container to collect all your urine over a 24-hour period.
    o Start the collection in the morning: Discard the first urine of the day, then collect all urine for the next 24 hours, including the first urine of the next morning.
    o Keep the container refrigerated or on ice during collection.
