The Endomysial Antibody-IgA (With Dilution) – IFA Test is a blood test that detects IgA antibodies against endomysium, which is a connective tissue surrounding muscle fibers. The test is done using Indirect Immunofluorescence Assay (IFA), which is a highly sensitive and specific method to identify these antibodies.
The presence of endomysial antibodies (EMA) is strongly associated with celiac disease, an autoimmune disorder where the ingestion of gluten leads to damage of the small intestine. The IgA form of the antibody is the most commonly detected in individuals with active celiac disease.

    Why It’s Done

Doctors may order the Endomysial Antibody-IgA (With Dilution) – IFA Test to:
    • Diagnose celiac disease, especially in individuals who present with symptoms like diarrhea, weight loss, abdominal pain, or bloating, and in cases where other tests (like tissue transglutaminase IgA) are inconclusive.
    • Confirm the diagnosis of celiac disease in patients who have gastrointestinal symptoms and a family history of the condition.
    • Monitor individuals on a gluten-free diet: The test can be used to assess whether someone with celiac disease is following the appropriate diet, as the antibody levels typically decrease when gluten is eliminated.
    • Evaluate suspected cases of gluten intolerance or wheat sensitivity, though this is less common.

    Preparation

    • No special preparation is usually required for this test.
    • Inform your doctor about any medications you are taking, especially if you are on a gluten-free diet or medications like immunosuppressants that may influence the results.
    • The test involves a blood sample taken from a vein in your arm.
