The Apolipoprotein B (ApoB) Test is a blood test that measures the level of ApoB protein in your bloodstream. ApoB is the main protein component of atherogenic lipoproteins such as low-density lipoprotein (LDL), very-low-density lipoprotein (VLDL), and intermediate-density lipoprotein (IDL) — all of which are involved in the buildup of plaque in arteries.
Each LDL particle contains one ApoB molecule, so this test provides an accurate count of the number of potentially harmful cholesterol-carrying particles, making it a stronger predictor of cardiovascular risk than LDL cholesterol alone.

    Description

    Why It’s Done:

    The ApoB test is used to:
    • Assess the risk of cardiovascular disease (heart attack, stroke, atherosclerosis)
    • Evaluate lipid disorders, especially when LDL-C and non-HDL-C levels are borderline or misleading
    • Guide and monitor lipid-lowering therapy (such as statins)
    • Help identify patients with familial hypercholesterolemia or metabolic syndrome
    Doctors may order this test if you:
    • Have a family history of early heart disease
    • Have diabetes, obesity, or high triglycerides
    • Are on treatment for high cholesterol and need detailed risk assessment

    Preparation:

    • Fasting is usually not required for this test, but some doctors may recommend fasting for 8–12 hours if it’s done along with a complete lipid panel.
    • A blood sample is taken from a vein.
