The Anti-Transglutaminase Antibody (IgG) test measures the levels of IgG class antibodies against the enzyme tissue transglutaminase (tTG) in the blood. Tissue transglutaminase is an enzyme involved in the modification of gluten peptides.
This test is especially useful in detecting celiac disease in patients who have IgA deficiency or when IgA-based tests are inconclusive. Elevated levels of anti-tTG IgG antibodies suggest an immune reaction to gluten, which damages the lining of the small intestine.

    Why It’s Done:

    This test is ordered to:
    • Help diagnose celiac disease, particularly in patients with low IgA levels
    • Monitor adherence and response to a gluten-free diet in diagnosed patients
    • Evaluate patients with symptoms such as chronic diarrhea, weight loss, anemia, or unexplained nutrient deficiencies
    • Screen individuals at risk of celiac disease when IgA-based tests cannot be used

    Preparation:

    • No special preparation is required.
    • Blood is drawn via venipuncture from a vein in the arm.
    • Continue eating a normal gluten-containing diet before the test; stopping gluten before testing may result in false-negative results.
