The H. pylori culture test is a laboratory method used to isolate and grow Helicobacter pylori bacteria from a gastric biopsy specimen (taken during an upper endoscopy).

• The specimen is placed on selective culture media under microaerophilic conditions to encourage the growth of H. pylori.
• Once cultured, the bacteria can be further tested for antibiotic susceptibility, helping guide effective treatment.

    Description

    Why It’s Done (Purpose):

    1. Confirm H. pylori Infection:
o Though less commonly used than breath, stool antigen, or blood antibody tests, culture is highly specific and confirms active infection.
    2. Antibiotic Sensitivity Testing:
o Especially important in patients who have failed prior H. pylori treatments, to identify resistant strains and guide tailored therapy.
    3. Research or Diagnostic Clarification:
o Sometimes used when non-invasive tests are inconclusive or histopathology suggests infection but requires confirmation.

    Preparation Guidelines:

    • Pre-procedure preparation for endoscopy is needed since the culture is done from a gastric biopsy:
o Do not eat or drink for 6–8 hours before the procedure.
o Stop antibiotics or bismuth compounds (like Pepto-Bismol) 2–4 weeks before the test if advised by your doctor.
o Stop proton pump inhibitors (PPIs) like omeprazole or pantoprazole at least 1–2 weeks prior, if medically appropriate.
    • Always follow your doctor’s instructions, especially if you’re taking medications for acid reflux or stomach ulcers
