The ANCA IFA test detects the presence of anti-neutrophil cytoplasmic antibodies in the blood using the indirect immunofluorescence assay (IFA) technique. ANCAs are autoantibodies directed against proteins in the cytoplasm of neutrophils (a type of white blood cell).
There are two main patterns detected by this test:
• c-ANCA (cytoplasmic pattern): Usually targets proteinase 3 (PR3), associated with granulomatosis with polyangiitis (Wegener’s granulomatosis)
• p-ANCA (perinuclear pattern): Usually targets myeloperoxidase (MPO), associated with microscopic polyangiitis and other vasculitides

    Why It’s Done:

    This test is ordered to:
    • Diagnose and help monitor vasculitis disorders, such as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA)
    • Evaluate symptoms such as unexplained inflammation, kidney problems, respiratory issues, or systemic symptoms that suggest autoimmune vasculitis
    • Assist in differentiating types of autoimmune diseases involving blood vessels

    Preparation:

    • No special preparation is needed.
    • Blood is drawn from a vein, typically in the arm.
    • Inform your healthcare provider about any medications or conditions that might affect immune response.
    • Fasting is not required.
